About SculptRx

With more than 20 years of nursing experience, Carly brings a strong clinical foundation and a calm, thoughtful approach to aesthetic medicine. Her background spans medical-surgical nursing, rehabilitation, long-term care, and home care, and most recently, corporate leadership and consulting work in clinical reimbursement and case management across Massachusetts, Connecticut, and Rhode Island.

Aesthetics brought Carly back to what drew her to nursing in the first place: caring for people directly. After years in the field, much of it in leadership, she now works one-on-one with each patient — and her goal for every visit is simple: that you leave feeling confident, comfortable, and happy with your results.

Carly completed Clinical Aesthetics Skills training through Boston College’s Connell School of Nursing, with training in neuromodulators, HA (hyaluronic acid) dermal fillers, biostimulators, and collagen induction therapy. She is also trained on the specific systems she uses at SculptRx, including Fotona®, SkinPen®, and T-Shape 2. Carly is a certified Laser Safety Officer and a member of the American Med Spa Association. She works with a dedicated Compliance Officer to ensure the practice meets the highest standards of safety and care. As a registered nurse, she is committed to continued education and plans to become a nurse practitioner.

As Founder and Lead Aesthetic Provider, Carly personally performs every treatment at SculptRx, combining clinical precision with a thoughtful, individualized approach so each patient feels comfortable, informed, and cared for.

Alexa Nicholls Costa, NP, is a board-certified nurse practitioner, founder of LexRx, and Medical Director of SculptRx Body & Aesthetics. She brings extensive experience in aesthetic medicine, emergency medicine, clinical education, and practice leadership.

Her background includes emergency medicine training through Brown Medical School and years practicing in the Emergency Department at Massachusetts General Hospital, alongside building LexRx into a multi-location practice across Massachusetts. She is founding faculty for Evolus Academy, faculty for Galderma’s GAIN program, and a recognized educator and speaker in aesthetic medicine.

As Medical Director, Alexa provides clinical oversight and supports safe, evidence-based treatment planning, protocols, and compliance. Her guidance helps ensure that care at SculptRx is delivered with sound clinical judgment and attention to each patient’s individual needs.
